Immediate Family Member Library Card Procession Guideline | National Tsing Hua University Library

Immediate family members of NTHU staff or professor emeritus are allowed to apply for a library card. Only the spouse or an immediate family member over age 6 a can apply for an Immediate Family Member Library Card.
How to apply
1. Location: Main Library First Floor Information Desk (Counter 3)
2. Required Documents:
  - Staff ID or Professor Emeritus ID
  - Family member's National ID card or other valid identification
  - One photo of the family member
  - Completed application form
3. Fee: NT$500 per card
4. Validity: Immediate Family Member Library Cards are valid for five years from the date of issue. They may be renewed free of charge by presenting the expired card.
5. If a staff member's or professor emeritus's status ends, all associated Immediate Family Member Library Cards will become invalid, and borrowing privileges will be terminated immediately.
If the photo on a Immediate Family Member Library Cards does not clearly identify the cardholder or raises concerns about the holder's identity, the Library may require the cardholder to reapply for or replace the card at no charge.
The cardholder of this library card must abide by the "National Tsing Hua University Library Reading Services" and the "Rules for Circulation Services".
The borrowing rights of the cardholders of this card is according to the "Rules for Circulation Services". For library equipment and rooms & spaces, please refer to respective regulations.
This card may only be used by the cardholder. If lost, please report to the library. A reissue costs NT$200.
